Attention: As of January 1st, 2022, we will be requiring a minimum of a $5 donation per individual for genealogy. When the form is submitted, we will first see if your loved ones are interred here at Forest Hill Cemetery. If they are, we will provide the plot and lot numbers. If you are looking for more information (ie. obituaries, letters, photos) we will require the donation before we release the records.
